Employee pulse survey template
A pulse survey trades depth for frequency. It asks the same handful of questions every week or month, so what you read is the change, not the level. This template is five questions and takes under two minutes, which is what makes repeating it possible.
The survey beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.
A survey that closes itself
A closing date stops the survey accepting responses, and anyone opening the link afterwards is told it has closed instead of submitting into a finished round.
Ratings and scales that average
Mood is a five-star rating and workload a labelled scale. Both report an average on the Results tab, which is the single figure worth carrying from one round to the next.
A conditional blocker question
“What is blocking you?” appears only for someone who says they are blocked. The rest of the team finishes a question sooner.

- Press “Use this template” and keep it to five questions. The format stops working the moment it stops being quick.
- Name the survey with its period — “Pulse, week 14” — because each round is its own survey with its own results.
- Under Settings → Advanced set it to stop after a date a few days out, so a round closes and the next can begin.
- For the next round, duplicate the survey from your dashboard, rename it and share the new link.
- Compare the averages on the mood and workload questions round over round; read the open answer for the reason behind a move.

Letting it grow
Every team adds “just one more question”. By the tenth it is an engagement survey that runs weekly, and the response rate tells you so.
Reusing one survey for every round
Results are tallied per survey. Collect week 2 into week 1’s survey and you can no longer tell the weeks apart.
Reacting to one round
A single dip is noise as often as signal, especially in a small team. Look for two rounds moving the same way before changing anything.
Never closing the loop
A pulse nobody mentions afterwards teaches people their answers go nowhere. Share one line of what you saw and one thing you will do, every round.
Questions about this survey
How often should a pulse survey run?
Often enough to see a trend and rarely enough that people still answer it — every two to four weeks for most teams. Weekly works only if it stays under a minute and someone visibly acts on it.
How do I compare one round with the last?
Each round is a separate survey, so each has its own Results tab with its own averages. Duplicate the survey for a new round rather than reopening the old one, or the two periods merge into one set of results.
Should a pulse survey be anonymous?
For morale and workload, yes — you get truer answers. This template asks for no name or email. If you want to follow up on blockers individually, add an optional name question and say plainly that it is optional.
Can I schedule it to open later?
Yes. Settings → Voting & results → “Opens at” holds the survey closed until the time you set, so you can share the link ahead of a Monday start.
