Employee survey templates

People answer an employee survey honestly when they believe it cannot be traced to them and that something will change. These templates ask for no name or email, keep results visible only to you, and group their statements by theme so the findings point at something a team can actually fix.

Statements that share a scale sit in a matrix, which keeps a twelve-statement survey to three screens. Each can close itself on a date you set, and one response per browser can be enforced without asking anyone to sign in. They collect opinions; they do not replace your HR system or a conversation.

If you are taking the temperature of the whole company

If you want a trend rather than a snapshot

Start from the employee pulse survey. Five questions, under two minutes, designed to be duplicated and re-run so you can compare one round with the next.

If someone is leaving

Start from the exit interview survey. The main reason is a single choice, so twenty leavers become a bar chart rather than twenty paragraphs.

If you just ran a session

Start from the training feedback survey. Alongside the ratings it asks whether the level was right and whether the attendee will use what they learned — the two answers that predict whether the training worked.
