Every template below is a real Votense form, not a screenshot. Open one, type into it, submit it, and see exactly what the person answering your survey would see. Nothing is saved — and when you are ready, one press opens it in the builder with every field already configured.

What every template here includes

These are not stripped-down demos. Each one is built from the same field types and settings you get on the free plan, so anything you see working here works in a survey of your own.

Validation that runs before submitting

Required fields, email formats and number ranges are checked in the browser, so a respondent fixes a mistake while they are still looking at it.

A link anyone can open

Every survey gets its own URL and needs no account to answer. Add a password in settings if a survey should not be open to everyone.

Responses you can take with you

Answers land in your dashboard and in any destination you add — your own webhook endpoint, or a Slack or Discord channel on a paid plan. CSV export is always available.

Put any of these on your own website

A survey does not have to live behind a Votense link. Two lines of HTML put it inside your own page, running in an iframe so your styles cannot break it and it cannot break your page.

It resizes itself

The script measures the survey and grows the frame to fit, so there is no inner scrollbar and no height for you to guess at.

It works without scripts too

Hosts that strip script tags — Notion, some website-builder tiers — can use a plain iframe with a fixed height instead.

Nothing to configure first

A pasted snippet works immediately. Restricting which sites may embed the survey is optional, and off until you ask for it.

Questions about the templates

Are these templates live, or just screenshots?
They are live Votense surveys. You can type into any of them and submit, and the validation behaves exactly as it would for a real respondent. Nothing you enter on a template page is saved — the submission is discarded rather than stored against the survey.
Can I use one of these as a template for my own survey?
Yes — press "Use this template" on any template and it opens in the builder with every field already configured, so the only thing left to do is press Save. Nothing is saved until you do. You can try any template on its own page first without signing in.
Which template should I start from?
Pick by what has to be true after someone submits, not by the name. Start from RSVP or event registration if the answers have to add up, the order survey if a wrong answer costs money, the job application survey if you are comparing people against the same questions, and the feedback survey if you need a number you can chart.
Do I need a paid plan to build these?
All 30 templates can be built on the free plan, which gives 10 surveys at 100 responses each. One detail is paid: the CV upload on the job application survey needs file uploads — every other field type used across the gallery is free. Delivering responses into Slack, Notion, Airtable or Google Sheets also needs connected apps. Webhooks, embedding and CSV export are free, so a free survey can still reach a system you own.
Can respondents fill in my survey without an account?
Yes. Only the person building a survey signs in. Whoever answers it opens a link or an embed and submits — no account, no sign-up step, and nothing to install.
