Product feedback survey template
Product feedback is easy to collect and hard to use, because open questions return a hundred different sentences. This template keeps the countable parts structured — how often, how good, which features, in what order — and leaves one open question for what the structure cannot capture.
The survey beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.
A ranking question
Respondents drag the options into order, or move them with the arrow buttons or the keyboard. The answer is stored as an ordered list and the results show each option’s average rank and how many people put it first.
Multiple choice that is tallied per option
The “which features do you use” question accepts several answers. Results give a percentage for each option out of the people who answered — not out of the total ticks — so 80% means four in five respondents use it.
A star rating with an average
The overall rating is a five-star field. Results show the count at every star and the average, which is the figure to track release over release.

- Press “Use this template” and replace the feature names in both the “which do you use” and the ranking question with your own. Keep the two lists identical.
- Limit the ranking to six items or fewer. People rank the top three carefully and the rest at random.
- Decide who should answer and say so in the description — current users, trial users, or people who left.
- Set results to “Only me” under Settings → Voting & results unless you want customers to see what others asked for.
- After collecting, read the ranking card on the Results tab: options are sorted by average rank, with how often each was put first.

Asking users to design the product
“What should we build?” gets feature requests, not problems. The ranking covers priorities; use the open question for what they were trying to do when they got stuck.
A ranking list that is too long
Beyond six options the lower half of every ranking is noise. If you have twelve candidates, run two surveys or use the feature prioritization template, which uses points instead.
Only hearing from your happiest users
People who love the product answer surveys about it. Send the link to recent cancellations and inactive accounts as well, or the results describe a product nobody is leaving.
Different feature names in different questions
If “Reports” in one question is “Analytics” in another, you cannot line the answers up. Use the same wording throughout.

How is a ranking question scored?
Each option gets the average of the positions people gave it, so a lower number is better. The Results tab sorts options best-first and also shows how many respondents ranked each one first.
What if someone does not move anything in the ranking?
An untouched ranking stores no answer rather than the default order, so the starting order cannot bias the result. A respondent who agrees with the order shown presses “Keep this order” to submit it deliberately.
Can I ask different questions to people who rarely use the product?
Yes. Open a question’s settings and add a condition such as “show only when How often do you use it is Rarely”. A condition matches one answer from an earlier choice, scale, rating or NPS question.
How do I get the answers into a spreadsheet?
Export CSV from the responses screen. Each response is one row and each question one column; a ranking is written as “1. Search; 2. Reports; …” in rank order.
