Education survey templates
A course evaluation is only comparable with another when both ask the same statements on the same scale. These templates keep the comparable part in agreement matrices and the course-specific part in open questions, so a department can line courses up while an instructor still learns something particular.
Students need no account and are asked for no name or student number. One response per browser keeps the count honest, and results can be released to students once the evaluation period closes — charts only, never the written comments.

Each one is a working form. Open it, fill it in and submit it — nothing is saved — then build it in your own account with every field already configured.
Which education template should you start from?
These differ less in their fields than in what has to be true after someone submits. That is the useful way to choose.
If you are evaluating a course
Start from the course evaluation survey. Statements on content and teaching use one agreement scale, so results are comparable between courses that use the same template.
