Workplace safety quiz

Eight questions on the safety basics that apply in almost any workplace — fire, manual handling, protective equipment, spills, chemicals and first aid. It is a starting point: adapt the answers to your own site rules and local regulations before you use it as a record of training.

The quiz beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.

A record of who passed

Every self-paced attempt stores the name, the score and each answer, and the results page lists the top scores and the questions most people got wrong.

Explanations that teach

Each wrong answer is followed by the right one and a short explanation on the score screen, so the quiz reinforces the training rather than just grading it.

How to use this template

4 steps in the editor. Nothing here needs a paid plan.

  1. Press “Use this template”, then read every question against your own procedures — fire assembly points, reporting routes and first aid arrangements differ between sites.
  2. Share the link after a training session with “One attempt per person” on. Each attempt is stored with the person’s name and score.
  3. To run it live, open the quiz and press Host live. Put the host screen on a projector: players go to votense.com/play on their phones, type the six-digit PIN and pick a nickname. You press Start; each question shows a countdown, reveals itself when time is up or everyone has answered, and a leaderboard follows.
  4. Export the responses as CSV from the results page to keep alongside your training records.

What goes wrong with this quiz

Specific to this template, not general advice about quizzes.

Generic answers on a site with its own rules

If your site uses a specific extinguisher type, alarm procedure or reporting form, say so in the question. A quiz that teaches the generic answer can contradict your own procedure.

Showing answers before everyone has taken it

With answers shown after submitting, early finishers can share them. Turn answers off until everyone has finished, then turn them back on.

Questions about this quiz

How many people can play at once?

Up to 50 players in one live game on the free plan (300 on Basic, 1,000 on Advanced), each on their own phone, with no account or app. Taken from a link instead of live, the limit is the plan’s response limit per quiz.

Does passing this quiz make someone certified?

No. It checks understanding after training; it is not a certification or a legal record of competence. Use it alongside the training your regulations require.

Can I set a pass mark?

There is no automatic pass or fail. The score screen shows the percentage and the results page shows every attempt, so you can apply your own pass mark when you review them.
